Some would argue that Ronnie James Dio came up with that or at least spread it to the Rock scene. It was a variation of something his grand ma did to put the hex on people I believe.

It depends on the context. Mano cornuto is an Italian magickal gesture. To fans of satanic and death rock it can be called devil's horns. To fans of rock that isn't Satan-focused it usually just means "rock on" (I think) and is often accompanied by head banging. Among Texas Longhorns supporters it is called "Hook 'em Horns".
